Sabina Jakubowski
Dental Assistant
Growing up in Phoenix has given Sabina her drive to help people in her immediate community. Sabina is a dental assistant working with Dr. Toal and Dr. Radeka at the Center for Dental Rehabilitation. She graduated Cum Laude from the University of Arizona with a B.S. in Health Sciences and a minor in German Studies as a first-generation college student and is planning to go onto dental school to become a dentist. She had been trained under Dr. Toal in assisting with patients compassionately and comprehensively through the Toal Method and 6 handed dentistry. Sabina serves to create a respectful arena dealing with a multitude of patients with a variety of needs, as each patient deserves quality care and unconditional understanding.
Lynn Kelly
Patient Coordinator
Lynn began her career in dentistry in 1978 as a clinical assistant, where she quickly discovered a deep passion for patient care. With decades of experience in the dental field, she brings both clinical insight and a compassionate approach to her role in administration.
As one of the first points of contact at the office, Lynn plays an essential role in creating a warm, welcoming, and supportive environment for every patient and their family. She is dedicated to building meaningful relationships and guiding patients through each step of their visit with attentiveness, empathy, and professionalism.
Lynn takes great pride in being part of a practice that uniquely integrates medical and dental services, allowing patients to receive comprehensive care in a comfortable setting. She values her role in helping patients feel at ease and supported throughout their experience, and she is honored to contribute to the high standard of care the office provides.
Chad Martin
Clinical Coordinator
Chad has been a valued member of our practice since 2003, bringing decades of experience and dedication to patient care. A proud U.S. Navy veteran, he served five years as a dental technician before transitioning into private practice.
Originally from Oregon, Chad moved to Arizona after his military service and has been committed ever since to providing safe, predictable, and high-quality care to every patient he meets. He takes pride in working alongside a team that feels like family and strives to create a comfortable, trustworthy environment for all.
Outside the office, Chad enjoys spending time with his family and playing with his dogs.
Dennis
Dental Assistant
Denis is a dedicated dental assistant with over a decade of experience, known for his calm presence and commitment to exceptional patient care. With 10 years in the field, he brings both skill and efficiency to every appointment, helping ensure a smooth and comfortable experience for each patient.
He was drawn to dentistry by a desire to make a meaningful difference in people’s lives, and he takes pride in supporting patients through every step of their visit. Whether assisting during procedures or helping patients feel at ease, Denis is an integral part of the team.
Outside of the office, Denis enjoys unwinding with his favorite TV show, Dexter, and is always looking forward to his dream vacation in Hawaii. When it comes to food, Chipotle is his go-to favorite.
Denis is passionate about providing a positive experience and looks forward to helping you feel comfortable and cared for at every visit.
Faith
Dental Assistant
With 13 years of experience as a Dental Assistant, Faith is passionate about providing compassionate, high-quality care and helping patients feel comfortable and confident in their dental experience. She takes pride in supporting her team, staying organized in a fast-paced clinical environment, and continuously expanding her skills to better serve patients and the community.
Outside of the office, Faith is a proud mother of two daughters who keep her busy with their many sports and activities. She enjoys spending quality time with her family, staying active at the gym, and finding relaxation in her newfound interest in planting. These interests help her maintain balance and bring positive energy to both her personal life and professional role.
Lynn Kelley

My passion for dentistry began in 1978 with my first job as an assistant. I have had privilege of working with some amazing dentists throughout my career. I am honored to be apart of Dr. Toal’s team here at the Center for Dental Rehabilitation. Dr. Toal’s method of treating patients has elevated me to a higher level of knowledge with dental rehabilitation.
